2023 Renewal Applications can be accessed by logging into your Member Profile.
To access your Member Profile:
- Navigate to any page on the PACS website, select 'Menu' in the top right corner and then select 'Member Login'. You can also click this link.
- Log in with your PACS # and password. If you can't remember your password you can click the "Forgot my password" link or you can contact the PACS office.

Descriptions of each industry can be found below. Organic Plans for each industry are accessible through your Member Profile. If there are other forms you require, typically they will be automatically added to your renewal application in the Online Renewal Portal in your Member Profile. However, if you require other Additional Forms they can be found in the Forms Directory. Please contact the PACS administration if you are having trouble accessing your online forms.
Crops
The Crop Renewal form is for operations that are already certified with PACS for crop production. These operations grow crops for human consumption, or for livestock feed for off site sales. If feed is grown onsite for your own livestock only, this information will be captured in the Livestock Renewal Plan. If your operation includes crops grown to maturity in containers within greenhouses, or the production of sprouts, shoots or microgreens, please complete a *Greenhouses, Sprouts & Microgreens* renewal (if you are already certified for Greenhouse, Sprouts & Microgreens production).

Livestock
Livestock Renewal:
The Livestock Renewal form is for operations that are already certified with PACS for livestock production. All renewing livestock operations must fill out the main Livestock Renewal form, as well as the appropriate Appendix according to the species’ you raise. Note: A renewal is not currently available for Apiculture operations, therefore a full plan must be completed at renewal time.

This renewal form is for operations that are already certified with PACS for mushroom production.

Preparation
Operators involved in food processing, preparation or packaging should complete the Preparation Plan. Brokers, traders or marketers who do not wash, process, package or label products should not use this form. Definitions are contained in the Preparation Guide. Additional forms that may be required to accompany the Preparation Plan can be found in the Forms Directory.
